for mailing or faxing required reports listed above to
Team Women on a timely basis:
1.
Attendance Report:
Mail or fax at end of every month
2. Referral Report: Mail or fax at end of every
month
3. Monthly Chapter Report: Mail or fax at end of
every month
4. New Officers Report / Change of Meeting Sheet:
Mail or fax after every election
5. New Member Report: Mail or fax after every
meeting
6. Resignation / Termination form: Mail or fax
only upon each occurrence
7. Chapter Dues Report: Mail or fax only
upon occurrence
